This 1-hour webinar will help health care providers improve their patients’ transition by learning the differences between pediatric and adult care to address and communicate those changes. Experts will share what pediatric specialists need to provide adult providers and how to help set expectations for the transition. Participants will also learn what adult physicians need to know about the pediatric care team, how to help patients build an adult care team, and how to support their patients in navigating the new adult system.
